When cancer cells are destroyed with the help of anticancer drugs or cytotoxic drugs then the treatment method is known as chemotherapy. Chemotherapy is said to be a systemic therapy as the drugs used in chemotherapy travel through the bloodstream to reach and destroy cancer cells present everywhere in the body. The duration of chemotherapy varies from every three weeks to two weeks.

Chemotherapy has its application at different times based on the treatments given to a patient. Basically it is used to treat cancer from preventing it to reoccur by destroying the cancer cells which have been left after surgery. It is used for relieving pain caused by cancer at a particular location and also to shrink a large tumour when it has not spread to the nearby surroundings, before removing it through surgery. Women who show a higher risk of recurrence of cancer are usually suggested for chemotherapy and this risk of recurrence depends on how far the cancer has spread, its hormone receptor status and the size as well as the grade of the tumour.

In chemotherapy different drugs and combination of drugs are used to treat breast cancer which depends on the overall health of the patient and stage of the breast cancer. Early stage or locally advanced breast cancer are treated with combination of several drugs and most of them are given intravenously, these include AC-T – doxorubicin (Adriamycin) and cyclophosphamide (Procytox) followed by paclitaxel (Taxol), T-AC – paclitaxel or docetaxel (Taxotere) followed by doxorubicin and cyclophosphamide, TC – paclitaxel and cyclophosphamide, doxorubicin and cyclophosphamide followed by docetaxel, CAF (or FAC) – cyclophosphamide (as a pill), doxorubicin and 5-fluorouracil (Adrucil, 5-FU), FAC followed by docetaxel or paclitaxel, CEF (or FEC) – cyclophosphamide (as a pill), epirubicin (Pharmorubicin) and 5-fluorouracil, CEF followed by docetaxel or paclitaxel, EC – epirubicin and cyclophosphamide, carboplatin (Paraplatin, Paraplatin AQ) and docetaxel, docetaxel and cyclophosphamide, adjuvant capecitabine (Xeloda) after a neoadjuvant chemotherapy combination. When breast cancer has spread to other parts of the body it is known as 4tg stage cancer or advanced breast cancer. Usually for advanced stage drugs in combination is not preferred as it may cause severe side effects. The drugs which are used in single are doxorubicin, pegylated liposomal doxorubicin (Caelyx), paclitaxel, nab-paclitaxel (Abraxane), capecitabine, gemcitabine (Gemzar), vinorelbine (Navelbine), cyclophosphamide, carboplatin, docetaxel, cisplatin (Platinol AQ), epirubicin. But still some of the drugs can be used in combination CAF (or FAC) – cyclophosphamide, doxorubicin and 5-fluorouracil, CEF (or FEC) – cyclophosphamide, epirubicin and 5-fluorouracil, AC – doxorubicin and cyclophosphamide, EC – epirubicin and cyclophosphamide, docetaxel and capecitabine, gemcitabine and paclitaxel, gemcitabine and carboplatin. For HER2-positive women are given targeted therapy drugs which are trastuzumab (Herceptin), pertuzumab (Perjeta) and trastuzumab emtansine (Kadcyla) in combination with the chemotherapy drugs.


Side effects of chemotherapy includes nausea, hair loss, constipation, diarrhoea, infection, sore mouth, loss of appetite, low blood cell counts, cognitive changes, nervous system damage and fertility problems.
